TRAVEL WAIVER FORM

I acknowledge that for reasons not reasonably foreseeable at this time, these travel plans may be interrupted or canceled by the supplier that is providing them, a government entity, or other third party over which 29:11, Inc/theBridge Community Church. has no control. I further acknowledge that the supplier's own cancellation, re-booking and refund policies, subject to any applicable law that is now or may later be in effect, will govern my rights and remedies, including my right to receive a refund, in such an event.

By signing below, I hereby agree to hold 29:11, Inc./theBridge Community Church harmless and release the agency and its representatives from any and all liability for any damages, including but not limited to monetary losses, I may incur as a result of such interruption or cancellation of these travel plans.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 while traveling. Such exposure or infection may result in personal injury, illness, permanent disability, and possible death. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with 29:11, Inc./theBridge Community Church. I hereby agree to hold 29:11, Inc./theBridge Community Church harmless and release the agency and its representatives from any and all liability for any damages I may incur as a result of these travel plans and the activities carried out during travel.

I acknowledge that engaging in mission trip service entails both known and unknown risks that could result in illness, severe physical and emotional injury, paralysis, death, or damage or loss of property. These risks include, but are not limited to: accidents in the course of travel to and from the destination; risk of injury or illness commonly associated with construction, recreation or other missionary service activities (including the risk of negligent acts or omissions); and risks associated with living and working in regions with underdeveloped security, buildings, roads, sanitation, food, water, and health care services and facilities. I understand that such risks are inherent to and cannot be eliminated from mission trip service.

I understand that 29:11, Inc./theBridge Community Church mission trip service entails travel in countries and regions where there may be a risk of criminal or terrorist activity.

I am participating in travel and activities with 29:11, Inc. and theBridge Church purely on a volunteer basis. I hereby assume any such risks that might result from my participation and I unconditionally agree to hold 29:11, Inc. and theBridge Community Church, its officers, employees, or other agents harmless for any liability concerning my personal health and well-being, or any liability for my personal property that might be lost, damaged, or stolen while on a trip, related directly or indirectly to the above trip, whether such injury arises out of the negligence of the Trip Sponsor or otherwise.

If a dispute over this agreement or any claim for damages arises, the Participant (or Parent) agrees to resolve the matter through a mutually acceptable Biblically based alternative dispute resolution process. If the Participant (or Parent) and the Trip Sponsor cannot agree upon such a process, the dispute will be submitted to a three-member arbitration panel for resolution pursuant to the rules of the American Arbitration Association.
